Child's Emergency Form

Instructions: A profile form is required to be on file for all child participants. All children new to our studio must submit this information prior to attending first class. Existing students of the studio may submit a new form as information changes. We may require an updated form once per year. Thank you!

In the event of an emergency and you cannot be reached, please list the names of those you'd like us to contact. This should also be someone who can pick your child up from class. ID will be required at time of pick up.

Attendance Policy

We are happy to provide you with the number of classes you register for. However, classes will be forfeited if you are absent on your own accord. In the event that you need to miss a class or lesson, please call or email us at least 4 hours prior to the class. On the occasion that we have to cancel a class due to teacher illness, weather, or family emergency, we will notify you via e-mail, unless another form of contact is requested and a class extension will be scheduled.

Release of Liability and Media Release Policy

I/We, the parent(s)/guardian(s), in case of injury or accident of any kind at Scrap U & Artistry Too, LLC agree to not hold Scrap U & Artistry Too, LLC nor any of its employees or volunteers liable for its occurrence. I hereby release Scrap U & Artistry Too, LLC and its employees and volunteers from all claims or damage which may arise out of any loss or personal injury to my child while participating in Scrap U & Artistry Too, LLC programs. I hereby give Scrap U & Artistry Too, LLC permission to use images/video taken of my child in this program for future or promotional activities.
